Appointments
At your first visit, you will be asked to fill out paperwork regarding insurance and a nurse will take your health history. Please bring your insurance card, any x-rays and a list of medications you are currently taking. This will help expedite your visit.
We will do our best to keep our appointment schedule. However, please understand that not all patients require the same amount of time with the physician and that emergencies do occur, therefore delays are unavoidable. Your patience in these situations is greatly appreciated.
Insurance
Insurance is a method of payment for services provided by your physician. It is your responsibility to pay any deductibles, co-payments or other balances not covered by your insurance carrier. If you have any questions about insurance coverage, our office provides experienced staff to assist you.
At the time of your visit, we ask that you provide our office with copies of insurance cards and other current information.
- Co-payments are due at the time service is rendered.
- We accept cash, checks, MasterCard, Visa and Discover cards.
Worker's Compensation Claims
As a courtesy to our patients, the office will file your claims. The patient, however, through insurance and/or personal payment, is responsible for claims that are denied, unsettled or unpaid within 60 days.
Prescription Refills
Prescription refills are issued only during office hours. After hours, your records are not available to the physician on call. We ask that you call prior to noon the day before you need a refill to give your physician time to review your records and call the pharmacy.
Emergencies
During office hours, please call (563) 344-9292 and our office staff will assist you. After hours and on weekends, you will be connected with the Physician's Exchange and your message will be relayed to the physician on call.
